My truck has done the same, cough or stutter for the first minute and a half and then changes to a totally different mode, and idles well. It does not do it as bad anymore, and the things that I have done since are flush out the transmission and replace with new fluid, run a can of LubroMoly diesel purge through a short circuit of just the IP and fuel filter, and then replaced the filter. I also blocked off the EGR system. There is still somewhat of a stutter, but I think that is tied to air getting into the IP. I have replaced the fuel lines with diesel safe clear tubing, so I can see the bubbles enter, but still cannot for the life of me find where they are coming from.
