Help please from Bio users

Has anyone got the address to send the monthly HO930 form to? They forgot to send my Jan declaration and when I rang said a copy of Decembers with correct volume used was fine, but I only copy the declaration side of the form and not the side with the add. Of course the office is closed now so I can't ring again.

are you paying 47 or 27p Linda? Im paying 27 but after the thing I just sent back im not so sure what they will say.
