You should really post this in the parts wanted section, might get a better response.

This is not a simple bolt on mod as you will also have to change or modify the tailgate to do this. Try a search on the forums as this mod had been done before.

I've seen ads for swing-out spare wheel carriers in mags for a place called MDG Off Road. Don't know the url.

I've bought a few things from them and I think they're an excellent company but they are Jeep specialists and I'd be very surprised if they are selling spare wheel carriers for Toyotas

I spent best part of 12 months searching the UK for a supplier for a swing aside spare carrier, found that nobody does one so eventually had to wait for somebody to break a Import Surf so I could get one. Toyota don't make them or supply them to fit 2nd Gen models any more so the only way you'll get one is to keep your eyes open for a "breaker"
If you do find one you'll have to make some internal modifications to the body to support the weight of it when it is hanging open. This I did by copying the how the swinging hinges were shaped to fit around the corner of the bodywork and then made some plates up to fit inside the body. You have to do the same thing with the catch bracket.
I sympathise with your problem of locating one it is extremely difficult.
I fitted one to mine as I originally had a underslung spare and removed it to fit LPG tanks on instead.
I have posted a few pics for you to look at

How difficult was it to make brakets for the carrier to bolt to ?! have been keen on doing this to mine but never found anyone who has made the brakets. any info would be good.

            cheers

              It was easy enough
I copied the shape of the carrier's hinged brackets and fabricated some internal supporting brackets myself. Once I'd got the shape, I marked and drilled the holes in the bracket. I welded on captive nuts for ease of fitting, I welded a flat piece of steel to the top of the bent bracket to create a 90 degree type edge for strength (I did the opposite on the bottom one) I offered the carrier up to the body, marked and drilled the holes. Mastic'd the bracket inside the body and bolted it all up. This way the weight of the carrier is spread across the corner of the body.
The tail gate section is as easy. You have to remove all the covers and the window winder mechanism. I put the catch on the carrier and put it against the tailgate and maked the relevant holes. The tailgate is curved so a bit more heating and bending went into the internal plate/bracket to make it fit flush with the tailgate shape. Again this had captive nuts welded in place. This was mastic'd and bolted up and VOILA! it worked
All I can say is I haven't seen another UK 4runner with a swing out carrier on
As per usual I got so engrossed in doing it I forgot to take any pics - SORRY

You can get a swing away tire carrier that mounts on the towball, and you can also get one that mounts directly to the frame rails. No mods to the rear door necessary. Search on www.yotatech.com for more info.
