am I missing a setting or something? on full screen (21 inch monitor) this is grainy and slightly pixelated?

It's all relative I suppose better than utube hd from what Ive seen http://www.tubewatcher.tv/7852 liked this....

Not bad on 1080p here. Can still see squares but it's good quality.

have a look at BBC i-player (top gear is in HD) and compare that to these, and you will see what I mean, I have a calibrated monitor.

hd tube
