Simple CGI Form mailer

Has anyone got and easy to configure, CGI form mailer script?

I'm putting together my website for all things offroad in Oxfordshire, and want a simple form mailer script so peole can fill out a form and send it to me...

What OS? Are you wanting the cgi to have a smtp backend as well or use the system mailer?

I host on sleasy space - so not sure what they use, I think it's Win2K and it may be useful for the CGI to have it's own SMTP as I'm not sure whether sleasy have the SMTP service running for me to use..
