customs & excise form for veg oil

i cant for the life of me fathom it out, i can get someone to sign it easy i know my name and my address (but after a gallon of guiness this isnt so easy) but i dont have a business so im stumped there am i, i take it the trader?????? and the schedule i presume the address is mine the distiguishing mark is car reg and description is a PLG car???? someone please put me right on this wann get sorted out asap,

Providing you do have the EX103 and NOT the 103A you ARE the trader - I have been scouring the net to find the example form I used to fill mine in, but no such luck so far

I can help with HO930s but regretfully I cannot recall what I enterred for the EX103.

Is it worth calling the telephone number you dug out ?

Before you post it, may I suggest either scanning it, taking a pic of it, or taking notes of what is required at each stage so as to help others here ?

You may find that calling 0191 201 1752 and asking for help may be more productive - I got contacted by that office for further details when I applied.

right here is what you have to pu in and where
full name and address well even i could put this

nature of business just state it is a private road vehicle

witness's im sure you all can sort that one

ok the premis on the left hand side is your own full postal address

distinguishing marks is the vehical registration number

description intended use goes along the lines of
used as an alternative fuel in a PLG car for private use, a mixture of Diesel and vegatable oil used together to make a bio blend
