Been looking at some after market bits and pieces on various sites and see that the Surf is not shown but the 4 Runner is. Can I assume that the majority of the add ons will fit a 2nd generation Surf?

Yep.

2nd Gen 4 Runner body shape is identical except US 4Runners don't have the wide body option generally (I think the Highlander is the US equivilent of the widebody Surf).

Best thing to do is if there is anything specific then ask about that one specifically on here, I'm sure that most things shouldn't be a problem.
