Your neck, really. My mum gets aching legs from her lower back (sciatica). I 'lose' my shoulder and upper arm about once every six months from sleeping funny and doing my neck a mischief.

Like Colin said, go to an osteopath. The physio may be able to fix it if it's a muscular injury, but it sounds like you've got something long-term wrong. GPs know bugger all about injuries unless its bleeding or broken. Then they know to send you to hospital.
